Financial Policy

Caliper Wellness will bill upfront for all appointments.  Debit or credit cards are preferred forms of payment for appointments. Once payment for the appointment/prescription is made, Caliper Wellness is under no obligation to issue a refund for services.

New appointments will not be made if there are any outstanding balances for prior appointments.

If you are late or miss your appointment, you may be subject to a $50 fee. Patients understand that there are no refunds for services or products rendered. We cannot accept back used medications once they have been dispensed per state regulation.

Services must be paid for at the time of service.

Health insurance typically does not cover services provided at Caliper Wellness. If you want to seek insurance reimbursement, we would be happy to provide you with itemized invoices that you can submit to your insurance company.

Appointment /Cancellation Policy

All appointments should be kept as scheduled to ensure consistency in the treatment process and no gaps in medication refills. As a general rule, medication management patients must first be stabilized, and then seen a minimum of every 3 to 6 months, depending on the medication. 

Medication Policy

Medication renewal will occur during the medication follow-up visit with the prescribing provider.  No medications will be prescribed over the phone without an appointment. As a general rule, controlled substances will not be prescribed, except for in few and rare exceptions, and strictly at the provider’s discretion.  Any script for a controlled substance prescription that has expired requires a return appointment. Additionally, Illicit, and recreational substances, including cannabis and alcohol, interfere with the efficacy of many medications. The provider reserves the right to not initiate, or to ask for sobriety, prior to initiating certain medications if substance abuse is suspected or evident.

Caliper Wellness reserves the right to drug test patients prior to or during controlled medication management.  Adequately screening for substances such as illicit or recreational substances that may interact dangerously with medications being prescribed is essential to prevent severe injury to our patients.

NO REFUNDS on medications ordered from compounding pharmacies. Once the visit has occurred and medications are agreed upon by both the provider and the patient, the medications will be ordered.

Off-label use of medications (including, but not limited to: Semaglutide, Tirzepatide, Peptides, Testosterone, & Clonidine) may be prescribed by the provider. Any questions about the use of off-label medications will be answered during the visit before the prescription is given. The patient is encouraged to ask questions about the use of off-label medications so that there is a full understanding of the medication that is being prescribed.

Phone Policy

If you have questions regarding your medication or are having any side effects from your medication, please call/text the practitioner immediately.  If it is after-hours, you can leave a message that will be relayed to the provider, and they will respond as soon as possible.  Non-urgent phone calls/text and/or requests made after 3 p.m. may not be returned until the next business day.  In case of an emergency, please call 911 or go the nearest emergency room.

 

For Patients Seeking Hormone Therapy and Prescription Treatment

The patient is voluntarily requesting treatment with Caliper Wellness in regards to hormone replacement therapy or other treatment modalities as determined by a mutual decision between the patient and the medical provider even if hormone levels are considered to be in the normal range for the patient's age based on other medical society recommendations and guidelines.

Having an appointment with Caliper Wellness does not necessarily entitle the patient to be issued a testosterone prescription. Every individual is different and it is at the medical provider's discretion to issue a testosterone prescription.

Testosterone is considered a controlled substance. If prescribed, patient agrees to take the medications as prescribed and follow the medical provider's instructions. The patient will not sell or share their prescriptions to other individuals.

Treatments used at Caliper Wellness might not be considered a medical necessity. Treatments rendered are for the purpose of improving your quality of life through hormone restoration, nutritional and supplemental counseling, and possibly weight loss treatment.

Patients agree that if they are experiencing any side effects or become sick, they will follow up with my primary care provider or go to an urgent care or emergency department.

The patient acknowledges that Caliper Wellness is not my primary care provider unless I elect them so. The patient agrees to continue with routine care through my primary care provider and notify them of treatments prescribed at Caliper Wellness.

The patient understands that they must maintain follow-up appointments to remain on treatment. It is important that lab work is monitored regularly for safety purposes. It is important that Caliper Wellness manages the treatment and it is at their discretion to provide it.

Caliper Wellness is not responsible for performing prostate cancer screening, colon cancer screening, digital rectal exams, or other age-related preventive care. Patients agree to follow up with my primary care provider to obtain these screenings and will hold Caliper Wellness harmless if an adverse event occurs during treatment. The Patient will ensure that my primary care provider provides the results of such screenings to Caliper Wellness as this could change the treatment prescribed.
